Medical clearance for enrolment is issued by the ISG Nurses’ Office.
A clearance email will be sent to the parents of students in compliance with the medical requirements from the ISG Nurses’ Office.
To obtain a clearance email to start school, the following forms and vaccination records (from birth) must be submitted (by email) to the Nurses’ Office.
Medical Documentation Checklist:
- ISG Health Inventory
- ISG Medical Form (must be signed and stamped by a medical doctor)
- Vaccination Records since birth
